What to bring:
- Referral letter
- Medicare card
- Private health insurance card, if applicable
- Pensioner Concession or DVA card, if applicable
- Relevant recent imaging reports
- List of your current medications

For your convenience, you may wish to complete your Patient Registration Form before your appointment.
You can email the completed form to admin@drkejwang.com.au or bring a printed copy with you on the day of your appointment.
